NT-proBNP
A peptide released when the heart is under pressure or strain; used in the workup of heart failure.

When it is tested
Measured in the evaluation of breathlessness and suspected heart failure, in monitoring of established heart failure, and in some oncology-therapy cardiotoxicity surveillance pathways.
Reading results
Values rise with age and are affected by kidney function, atrial fibrillation, and obesity (which lowers values), so rule-out thresholds are age- and context-specific. A markedly elevated value supports heart failure but does not reveal its mechanism; a low value in an untreated patient is valuable for excluding it. Reference ranges are assay-specific and cannot be transferred between platforms.
